The only case I can think of where the .gov may be cheaper is the one where they actually provide the healthcare , the VA. Medicare is administered by private companies.

If insurance companies were to sell identical products to all comers (ie. no preexisting conditions, age discrimination, etc), that's about as cheap as it gets.

The idea here is to get a bit of free market going by getting them to sell identical products. They can fight over the efficiency of their back office.
